For your convenience, please bring with you any medical insurance cards with your policy numbers on them. If your insurance company requires a referral form from your primary care physician, you MUST bring them with you on the day of your appointment. PLease allow 1-2 hours for you examination. You may be dilated during your examination and your vision will be temporarily blurred, so you may want to bring someone with you who can drive.
